At 10:30 a.m. on August 15, 2018, Employee #1 was operating the W. E. Plemons Machinery Tray Box Former. While operating the machine, a flat box "blank" became jammed in the feeding section of the machine's shuttle carrier. In an attempt to clear the jam, the employee reached over a guard to free the jam with one hand, while operating the "jog" button with the other hand. The employee's gloved left hand became caught by the machine's shuttle carrier. Employee #1's middle finger was caught, causing injuries that required surgical amputation and hospitalization. | |||||||
