At 1:45 p.m. on August 10, 2018, Employee #1 was operating the Disc Master grinding machine. The point of operation was unguarded and only a stiff bristle brush was in place, which did not prevent contact. As the employee fed parts into the machine, a part became stuck and kicked back at the employee, causing an amputation of the left middle finger from the top knuckle. Employee #1 was hospitalized for his injury.
