At 2:15 pm. on August 13, 2018, an employee exited his forklift to scan a produc t code located on a rack. The employee was struck by a second forklift being op erated by a coworker who had been stacking a pallet and backed up. The employee was crushed between the two forklifts and taken to the hospital where he died f rom his injuries.
