At 3:00 p.m. on July 16, 2018, an employee was changing film in the Mira-Pax C Machine when he reached under the guard to help guide the film through the heat seal and shears. The employee pressed the stop button; however, the machine cycled itself twice more, amputating the left hand's middle fingertip.
