At 3:00 p.m. on June 5, 2018, an employee was using a table saw to cut flooring. The table saw's blade guard had been removed prior to the employee using it and while cutting a piece of flooring, the employee's left index finger and thumb contacted the table saw blade. The employee's index finger was amputated due to contact with the blade.
