At 11:30 a.m. on July 9, 2018, Employee #1 was operating a pump that was supplying water for an oil/gas fracturing operation located one mile away. The employee switched pumps, caught prime, and was ramping up the flow rate. Shortly after starting the second pump, the clamp securing the hose/elbow to a flange failed. The hose/elbow struck the employee's right shoulder, upper right back, and rear right pelvis. Employee #1 sustained two fractures to his pelvis, contusions, and abrasions. He was hospitalized for his injuries.
