At 10:45 p.m. on June 20, 2018, an employee was manually pulling on the drive be lts of a machine to clear a jam. The equipment was unguarded. The employee's lef t hand was pulled into the machine, resulting in the amputation of his left ind ex finger above its first knuckle. The employee was hospitalized and received tr eatment for his injury.
