At 9:05 a.m. on June 27, 2018, an employee was cutting sized portions from a blo ck (bale) of polymer with a bench-type bale cutter called the Spadone Cutter. T he employee reached into the cutting area of a bench type bale cutter with his l eft hand and was pulling a block of polymer in order to make the next cut. The employee leaned on the palm actuator button and the machine cycled, amputating h is left hands fingers behind the first knuckle and thumb at the distal joint. T he employee was hospitalized.
