At 8:15 a.m. on June 28, 2018, an employee was observing the operation of a bacon-slicing machine. During its operation, the employee placed his left hand into the machine's moving parts. The machine's die activated, catching the employee's left index and middle fingers. As a result, the employee's index finger was amputated at the first knuckle and the middle finger at its tip.
