At 7:10 a.m. on May 11, 2018, Employee #1 was operating the boom of a material handling truck. The employee lifted a cart and began off loading it. He extended the boom of the truck too far and the truck tipped up. As he dropped the cart, the truck shifted back to center. The employee fell out of the seat onto the asphalt, fracturing his elbow and right knee, a lung contusion, a minor brain bleeding, minor liver and colon bleeding, and a lip and chin laceration. Employee #1 was hospitalized for his injuries.
