At 8:00 a.m. on January 15, 2018, Employee #1, employed by a construction company, was installing foam insulation at a two-story residential structure. He was working from a 6-foot stepladder placed on the second floor and near a floor opening. Employee #1 lost his balance and fell from the ladder and through the floor opening to the ground floor, a fall height of 12.5 feet. Emergency services were called, and Employee #1 was transported to the hospital, admitted, and treated for fractures to his right forearm and right wrist.
