At 4:50 p.m. on May 21, 2018, Employee #1, employed by a roofing company, was sweeping debris from the roof of a one-story commercial building as part of a renovation project. He fell through a hole that measured 18 inches by 18 inches on top of the roof, a fall height of 14 feet. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for fractures to his right elbow and wrist.
