At 7:00 p.m. on May 14, 2018, an employee was loading a trailer with a corrugate d pipe while using a Caterpillar Model DP70N1 Forklift Truck and had secured a l oad on the truck's tongs before backing up. The employee started to back up wit hout looking and struck a parked forklift that was being unloaded by hand. The employee was struck in the chest by the second forklift's prongs when they came through the employee's cab. The employee was hospitalized with a crushed chest.
