At 10:10 a.m. on November 9, 2017, Employee #1, employed by an electrical services company, was engaged in electrical work at a water/sewer treatment plan. He attempted to use an STB Phase Rotation Indicator, Model Number 50109-G-DX2, to check the rotation on one of the plant's blowers. The blower was energized, and when Employee #1 connected the test equipment, he sustained a 4,160-volt electrical shock and electrical burns. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for the electrical shock and burns to both forearms and hands.
