At 8:00 a.m. on April 6, 2018, Employee #1, employed by a roofing company, was working at a multiemployer construction project, a one-story commercial building. He was on the roof and picking up debris. Employee #1 stepped on a piece of plywood that was covering a hole in the roof's surface. The plywood tipped, and he fell through the hole, a fall height of approximately 13 feet. Emergency services were called, and Employee #1 was transported to the hospital, admitted, and treated for a fractured arm.
