At 11:45 p.m. on February 2, 2018, Employee #1, employed by a construction company, was working at a four-story refinery. He was hanging lights on top of a reactor head in preparation for rigging. Employee #1 fell through an open 3-foot by 3-foot grating, a fall height of approximately 48 feet. As he fell, he made contact with a temporary scaffold that helped break his fall. Emergency services were called, and Employee #1 was transported to the hospital, where he was admitted and treated for jaw and facial fractures. | |||||||
