At 4:30 p.m. on November 3, 2017, an employee was loading materials into a dye w hen a piston dropped by itself and compressed the employee's fingers between the still cup and the cylinder shaft. The employee fractured her hand as well as a mputating two fingertips from her index and middle fingers. The employee was ho spitalized.
