At 10:00 a.m. on January 10, 2018, Employee #1, employed by an energy services company, was working on the construction of a tank. The weather was freezing, and he was placing wooden mats and tarp on top of the tank to prevent the tank from freezing. Employee #1 stepped outside of the handrail, slipped, and fell approximately 10 feet. Emergency services were called, and Employee #1 was transported to the hospital where he was admitted and received treatment for a fractured leg.
