At 5:30 p.m. on February 13, 2018, an employee was tightening and replacing a be lt on a drive motor of the air handling unit and had not locked out or tagged th e equipment. The employee reached into the belt and pulley area to check the te nsion and had his finger pulled into a pinch point, where he amputated the tip o f his right hand's middle finger. Hospitalization was not required.
