At 9:00 a.m. on February 1, 2018, Employee #1, employed by an equipment supplier , was working at a multiemployer construction project, a two-story commercial bu ilding. He was climbing down an extension ladder set against a 12-foot mezzanin e. The ladder slipped, and Employee #1 fell and struck the concrete floor. Eme rgency services were called, and Employee #1 was transported to the hospital, wh ere he was admitted and treated for a fractured disc.
