At 7:45 a.m. on August 17, 2017, Employee #1 was driving a forklift his normal d uty, and he saw a pallet of product was stuck on a conveyor. When the Employee # 1 saw Coworker#1 step onto the conveyor that was stopped, he stepped onto the co nveyor to assist in manually shifting a pallet of cartons into a better position . Coworker #2 was at the conveyor controlls and started the conveyor. Two conve yor sections usually operate in the same direction. One section was started in a reverse direction, two belts converge at a 1/2 inch gap. Employee #1's left fo ot was pulled onto the 1/2 inch gap between the two conveyors causing the crushi ng injury. Employee #1 sustained severe damage to his left foot. Skin from the top and bottom of the Employee #1's foot was scraped off and he had sustained b roken bones.
