At 8:00 a.m. on August 28, 2017, Employee #1 was line boring the lower bucket sl eeve on a CAT 992 loader using a CAT 9U5730 Portable Boring Bar Unit. He was ho lding an internal micrometer in the hole after the first pass, and needed to adj ust the horizontal position of the boring bar. As he tried to activate the hydra ulics with his other hand, he pulled up the wrong button on the line boring mach ine control pendant and started the drill that rotated the boring bar instead. T he drill was plugged into the hydraulic unit separately, but was not unplugged o r locked out. Employee #1 sustained open fractures to his right hand and lacerat ions to his right forearm. He was hospitalized for his injuries.
