At 2:40 p.m. on January 24, 2018, Employee #1 was performing maintenance activities of the metering cylinders located in the Packaging Robot cell on line #1. Employee #1 operating inside the cell, was performing an operational test of the metering cylinders escapement pins. Employee #1 was providing verbal instructions to a coworker who was manually initiating the cycle via solenoid control outside the cell. As Employee #1 was making adjustments to the float controls, his left third finger dropped between the rear plate of the escapement pin and the cylinders housing. The coworker initiated a cycle, and Employee #1's fingertip was caught between the two components. No effective form of guarding around the pinch point or isolation of pneumatic energy was in place at the time of the event. Employee #1 was hospitalized and treated for his injuries.
