At 3:00 a.m. on January 9, 2018, Employee #1 was troubleshooting the ink cylinder on a 5054 Flexo Printing Machine, manufactured by the Ward Machinery Company. The ink cylinder was guarded by a magnetic interlocking system that stopped the movement of the cylinders when it was opened. The magnetic interlocking system was broken at the switch, and was not working. Employee #1 attempted to discover if there was pressure being applied to the feeder device that was used to apply ink on the cylinder. As he opened the guard, he placed his fingers on the feeder to check it. The ink cylinders were still rotating and the dry screen caught Employee #1's finger between the dry screen and the ink roller. The employees finger was amputated to the first knuckle. He was hospitalized for his injury.
