At 3:15 p.m. on January 8, 2018, Employee #1, employed by an industrial cleaning company, was working at a multiemployer project. He was operating a 20K pump h ydro blasting gun to break up material that had accumulated in a 6,000-gallon ch emical reactor. Employee #1 lost his balance while standing on top of the loose material and shot his left foot with approximately 18,000 pounds per square inc h (psi) water pressure. Emergency services were called, and Employee #1 was tra nsported to the hospital, where he was treated for a fractured fifth toe and a l acerated tendon. He was released the same day.
