At 9:30 a.m. on November 7, 2017, an employee was straightening cans and inserting cardboard between rows of cans in a Whallon 300 Brite Stacker Palletizer. The employee walked around the machine guard to put a can in an empty spot on a previously placed layer of cans. Resting his left hand on a layer of cans for support, he used his right hand to place a can in the empty spot. The machine cycled down on top of his left hand, amputating his left middle finger tip.
