At 11:40 a.m. on October 31, 2017, Employee #1 was replacing ammonia supply lines. After replacing one of the supply lines, Employee #1 walked to the other side of the space to replace a second supply line. Simultaneously to Employee #1 traversing the work area, a coworker tossed a section of an ammonia supply line to the ground. Residual liquid ammonia was ejected from the line and splashed Employee #1 in the face and eyes. Employee #1 sustained second degree burns to his eyes and face and was hospitalized for his injuries.
