At 2:15 p.m. on November 10, 2017, Employee #1 was performing a test to determin e the proper drive belt tension of a slurry tank impeller/agitator. Without turn ing off and locking out the power, the employee removed the guard for the drive belt and tried to check the tension by reaching into the machine and pulling on the inner surface of the belt with his hand. The belt pulled his hand toward the in-running nip point of the drive pulley, amputating the distal phalange of his right index finger.
