At 6:45 a.m. on June 21, 2017, an employee was operating a dock plate and extend ed it onto a truck when it became stuck. The employee attempted to extend the do ck plate further and the plate folded over, crushing the employee's right middle finger and amputating the tip. No hospitalization was required.
