On June 28, 2017, an employee was lifting up a dock leveler or a foldable dock r amp on the loading door in order to deliver some goods from the delivery truck t o the warehouse using a forklift. The employee used his hands to lift the door rather than the prybars that were located on the forklift or at the supervisor's desk. The employee was unable to fully extend the metal dock leveler and it fe ll back onto its hinges, crushing the employee's small right finger beneath the distal joint in the metal hinge. The employee amputated his fingertip and was h ospitalized.
