OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to contact from, or struck-by hazards associated with automated guided vehicles used for product movement in the high density storage department without adequate protections, or methods to prevent contact related injury: a. Anheuser Busch, Columbus, Ohio facility: On or about March 5, the employer had not assured that Eagle II and Eagle III automatic guided vehicles (AGVs) did not have adequate protections to prevent contact related injury to employees performing required duties. While "reinserting" a faulted AGV back into the active path of operation, an employee was struck by another AGV and pinned between the two AGVs. The bumper stop systems utilized failed to protect the employee during this activity. As many as 15 AGVs were found to have issues with the bumper stop safety system of protection. b. Anheuser Busch, Columbus, Ohio facility: On or about March 5, the employer had not established an AGV inspection program to assure the operation of bumper stops as a protective measure to prevent employees from being struck by AGVs when performing required duties in the active path of operation. The safety switch components for the bumper stops were only inspected on a 2 month cycle when manufacturers recommendations call for a minimal weekly inspection (or inspection after 50 hours of service) of the bumper stop components to assure this protective measure is operational.
