29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a. On or about October 1, 2014, at the above addressed jobsite, employees accessing and egressing the roof were exposed to falls of approximately 9 feet as the ladder in use, which was set along the sloped side of the roof, only extended 1-1.5 feet above the roof line.
