29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): a) Jobsite, 8136 Ohio River Boulevard, Pittsburgh, PA: On or about October 1, 2014, an employee was working in a 3 foot wide area behind a trench box to put the recovery hose for the sump pump into the recovery well that was located behind the trench box. The employee was exposed to a cave-in hazard of approximately 6-10 feet of excavated soil that was not sloped back from the top of the trench box to the edge of the pavement.
