29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: Maintenance employees were exposed to caught-in/crushing hazards caused by incorrectly/incompletely locking/tagging out machinery throughout the facility. The maintenance employees, deemed authorized to perform lockout/tagout, did not receive adequate training: a) Machine specific, hands on lockout/tagout training was not given to each authorized employee; b) A demonstration of locking/tagging out the Beller on Line #5 revealed that the proper procedure was not followed.
