29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines: (a) Former Foundry Building, 4831 Front Mountain Road, Belleville, Pennsylvania - Employees worked to repair the roof deck and to install insulation panels on the western part of the building, 32 feet above the ground. To access this flat roof, employees used a fixed ladder at the east end of the 24-foot high roof, walked across the 24-foot high flat roof, and then used another fixed ladder on the 24-foot high roof to get to the 32-foot high roof. The employer had installed a warning-line around the work area on the 32-foot high roof but did not connect the points of access to the work area by installing warning lines up to the ladders and by using warning lines to form an access path across the 24-foot high roof between the two ladders, exposing employees to the hazard of falling to a lower level, on or about September 23, 2014. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
