29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation, and the operator does not have a clear and direct view of the edge of the excavation, a warning system shall be utilized such as barricades, hand or mechanical signals, or stop logs. If possible, the grade should be away from the excavation. On or about August 16, 2014, at the above addressed job site, a warning system was not used when a bulldozer operator fell into a 15 foot by 10 foot excavation of water while performing back dragging operations adjacent to the hole.
