29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a) 4290 Alatex Road (Plant 2) - production floor; On or about August 7, 2014 and at times prior thereto, the employer exposed employees to struck-by hazards in that the employer permitted operators of powered industrial trucks (PITs) who had been involved in an accident to continue to operate PITs without first providing the operator with retraining.
