Text For Citation: 02 Item/Group: 005 Hazard:

29 CFR 1910.146(j)(2): The employer shall ensure that the entry supervisor verifies, by checking that the appropriate entries have been made on the permit, that all tests specified by the permit have been conducted and that all procedures and equipment specified by the permit are in place before endorsing the permit and allowing entry to begin: a. On or about July 28, 2014, employees were required to enter permit required confined spaces. Employees were exposed to hazards associated with permit required confined spaces when the employer failed to ensure that the entry supervisor completed specified duties including, but not limited to, ensure required mechanical lifting devices and lighting equipment were in place and used prior to allowing entry to begin. The Dedicated TCS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146(j)(2), which was contained in OSHA inspection number 415682, citation number 1, item number 7 and was affirmed as a final order on November 20, 2012, with respect to a workplace located at 17356 Torrence Avenue, Lansing, IL 60438.
