Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Manko Window Systems, Inc. at 17500 E 22nd Ave, Aurora, CO 80011: On or before June 23, 2014 the employer did not receive the manufacturer's written approval for a modification or addition to the Hyster forklift, model S40FT, serial number F187V03363C. The lift truck was used to move loads that weighed approximately 4,000 pounds by using an 8-foot web sling that was attached to each tine. This condition exposed employees to the hazard of operating modified forklifts without the manufacturer's approval. (b) Manko Window Systems, Inc. at 17500 E 22nd Ave, Aurora, CO 80011: On or before June 23, 2014 the employer did not receive the manufacturer's written approval for a modification or addition to the Yale forklift, model GLC080LGNGAE085, serial number B818N02431Y. A WESCO JigRig is attached to the tines of the forklift and is used to lift and move crates that can weigh up to 4,000 pounds. This condition exposed employees to the hazard of operating modified forklifts without the manufacturer's approval.
