Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards: (a) Manko Window Systems, Inc. at 17500 E 22nd Ave, Aurora, CO 80011: On or before June 21, 2014 employees were exposed to crushing and struck-by hazards while unloading wooden crates of glass from an enclosed shipping container and were working within the fall zone of the crates. Employees were attempting to position the 4,000 lb. wooden crates for removal from the shipping container when two of the crates tipped over and crushed an employee; fatality injuring him. The employer did not ensure steps had been taken to protect employees from crushing and struck-by hazards. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: A feasible method to correct the hazard is to develop and implement a written procedure that prevents employees from being exposed to crushing and struck-by hazards while unloading wooden crates of glass from enclosed shipping containers. Safe work practices must be established to ensure employees remain clear of the fall zone while removing metal banding and that steps are taken to secure crates from falling over during the unloading process.
