Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard as a result of pipe over-pressurization. On or about June 19, 2014, during a water pre-flush operation at the Werner Well 29H1, the employer failed to protect employees from the hazards associated with a water flow pipe violently breaking away at a threaded attachment from the piping assembly due to the build up of hydraulic pressure within the line as water was introduced into the system but no outlet bleeder valve was opened to facilitate an open pathway for the water. The pipe struck and fatally wounded two employees working in the area, hammering on a leaking coupling.
