29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools for handling equipment when the tools or handling equipment might have made contact with such conductors or parts: On or about 13 June 2014 At Naval Base Ventura, Point Mugu, an employee working on T211 a 150KVA 4160-120/208V 3 Phase delta/wye dry pack transformer reached in through the high voltage side with a Ryobi 18 V Impact Wrench - Model P260 non-insulated drill and socket to secure the neutral line bolt that was within 12 inches from a live circuit exposing the employee to an electrical hazard that resulted in an electrocution.
