29 CFR 1910.333(a): "General." Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards: On or about 13 June 2014 at Naval Base Ventura, Point Mugu, an employee was working on a 150KVA 4160 - 120/201v 3 phase delta wye dry pack transformer (T211) and made direct contact with a closed circuit of T211 while reaching in through the high voltage side which had not been properly secured by power isolation, lock out tag out, and/or barricades.
