29 CFR 1910.132(d)(1): General Industry, Personal Protective Equipment: Hazard assessment and equipment selection: The employer failed to assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE): On or about 13 June 2014 at Naval Base Point Mugu the employer failed to perform a hazard assessment to determine the hazards present in the workplace which would necessitate the use of personal protective equipment (PPE). The hazard assessment would reveal that an employee working on a three phase 150KVA 4160 - 120/201v 3 phase delta wye dry pack transformer should have been required to use personal protective equipment including leather or insulated gloves with protectors and face shield. The employee was exposed to electrical hazards that resulted in electrocution.
