29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) Up to and including 8/10/2011, the Harris shear work crew responsible for changing shear blades and other regular service were not trained in the potential for immediate release of the clamp when the liner bunched up and potentially interfered with the sensor for the bypass hydraulic pressure system. They also were not trained in evaluating the potential hazard of interfering with safety systems when maintaining lockout overnight. b.) Authorized employees were not trained in the lockout checklist. They signed off on a lockout checklist that did not specify procedures performed, did not verify actions taken, and were incomplete in many instances (# employees did not match #signatures, only portions were checked off, etc.). Employees were not trained to complete the checklist as an affirmation that all elements of the lockout had been initiated.
