29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: a) Fabricated Scaffolding System located right of the residential structures front door: On or about January 22, 2014, the employer and/or designated competent person did not ensure the platform planks were erected and used with no more than 18-inches of extension over its respective supports which exposed employees to a fall of 18-feet to the ground below.
