29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Stockroom: Employees are exposed to falls of 12 feet from top shelves of storage racks to concrete floor below when they step onto top shelves in order to retrieve or store boxes. Employees do not wear personal protective equipment for fall hazards.
