29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about November 27, 2013, at the job site located on Sugarloaf Tower Rd, near Calico Rock, Arkansas, the employer had not provided training on the control of hazardous energy to employees who were performing maintenance on a Timberjack Model 840 tree feller/buncher An employee was replacing a hydraulic hose on the equipment with the cutting head elevated approximately 5 feet above ground level. Once the hose was removed, the cutting head fell on the other employee working on the equipment. This created conditions which exposed the employee to the hazard of being crushed by the falling head assembly.
