29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about November 27, 2013, at the job site located on Sugarloaf Tower Road, the employer did not develop procedures for the control of hazardous energy sources including mechanical, hydraulic, and gravity for employees performing maintenance on a Timberjack Model 840 tree feller/buncher. An employee was replacing a hydraulic hose on the equipment with the cutting head elevated approximately 5 feet above ground level. Once the hose was removed, the cutting head fell on the other employee working on the equipment. This created conditions which exposed the employee to the hazard of being crushed by the falling head assembly.
