29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about November 27, 2013, at the job site located on Sugar Loaf Tower Road near Calico Rock, Arkansas, the employer did not have in place an energy control program to ensure that employees performing maintenance on a Timberjack Model 840 tree feller/buncher were not exposed to the release of stored energy. An employee was replacing a hydraulic hose on the equipment with the cutting head elevated approximately 5 feet above the ground. Once the hose was removed, the cutting head fell on another employee working on the equipment. This created conditions which exposed the employee to the hazard of being crushed by the falling head assembly.
